Warranty Claim Form

Procedure:

1. You will need an RMA Number to submit this warranty claim. To request an RMA and a prepaid return label, contact DENSO Customer Care at 1-877-675-5463

2. Fill in all 9 pieces of “Required Information” below. Please complete one form for each Device Serial Number.

3. Proof of purchase, such as a dated invoice will be required, please upload a copy to this form.

4. After you submit your form, you will recieve a confirmation email with a PDF copy of the form. Please print this out and wait for a DENSO representative to contact you.

5. After you are contact by a DENSO representative, please send the defective part, a printed copy of the completed warranty claim form PDF you received in your confirmation email, and proof of purchase to:

6. If DENSO determines the defective part(s) are covered under the warranty, a replacement part, remanufactured or new, will be shipped to the end-user within 5 working days of the accepted claim date.
